Nomi Health and CHG Healthcare have announced a new partnership aimed at expanding medical support to underserved communities across the United States. Unlike conventional patient care models that often encounter obstacles, the collaboration between Nomi and CHG offers a seamless, integrated solution. This solution includes various components such as multi-channel patient engagement, virtual care access, and performance tracking metrics based on clinical and claims-based data.

In contrast to traditional approaches that create barriers to care, the Nomi and CHG partnership allows providers to initiate virtual visits with at-risk patients immediately upon engagement. Through text, email, chatbot, and live agents, patients can access telehealth consultations promptly, ensuring timely care and facilitating the establishment of support and case management throughout their healthcare journey. The program operates nationwide in all 50 states and offers services in multiple languages.

By adopting a comprehensive care delivery approach, the program effectively addresses gaps in care while utilizing data for ongoing analysis and real-time insights to monitor progress. Led by healthcare providers, this initiative aims to reduce overall care costs through actionable analytics and improve Healthcare Effectiveness Data and Information Sets (HEDIS) scores, thereby enhancing the quality and efficiency of healthcare delivery.
