TriValley Medical Group (TVMG) and Golden Valley Health Centers (GVHC) have announced a new collaboration, marked by TVMG acquiring a majority stake in Sequoia Health IPA, a joint venture between GVHC and a prominent Federally Qualified Health Center (FQHC) in California's Central Valley. Sequoia Health IPA serves over 145,000 patients, particularly focusing on vulnerable populations.
The partnership aims to improve healthcare access and outcomes by strengthening value-based care initiatives and optimizing operations. This involves integrating primary and specialty care networks, expanding services for patients with various insurance plans, and enhancing care coordination.
TVMG will provide expertise in contract management, care model development, technology integration, and data analysis to support GVHC. Leveraging TVMG’s advanced care platform and tools, the collaboration seeks to enhance GVHC’s network of healthcare providers.
The primary goal is to address health equity issues in the Central Valley community by delivering accessible, high-quality, value-based healthcare services. This aligns with CMS's objective to transition all Medicare beneficiaries into accountable care relationships by 2030, aiming to reduce hospital visits, provide cost-effective services, and improve patient outcomes through coordinated care and shared data.
